Student Growth Plan Template

Use this template to plan an individual student's growth across reading behaviors. Consider the student's current reading level (which will not necessarily correlate with grade level) and compare those behaviors to the next level. Use Fountas and Pinnell's "Behaviors to Notice" document as a resource. Choose only 1–3 focus skills to work on at any given time. Complete the initial plan now, then return to record status checks and the final exit notes on your scheduled dates.
